Exemplification is a certificate attesting that a document is a true record of the court. The certificate provides the Chief Judge's certification of the clerk’s status and the Clerk's certification of the chief judge's status.
Some state court's requires an Exemplification Certificate Director Form 1310 when a federal court certifies a document for filing with a state or local authority.
It is only necessary for the clerk to execute the form when a person requests that the Clerk complete the form.
Register a certified document in a Foreign Country: An Apostille is required to certify a document for filing in a foreign country. Only the district court clerk may issue an Apostille. |
Requirements:
Review Docket:
Verify fee paid (receipt docketed)
Certify Document:
Print document (judgment/order) from the docket sheet.
Include PDF header. Certified copies must include the EOD (entered on docket) date.
Always print the document from the docket. Do not certify document provided by the requestor.

Prepare Exemplification Certificate ("Certificate")
Prepare certificate (See local form MSSB-1310 in template folder)
Photocopy the certificate and certified document
Attach certified document to certificate
Mail document or deliver to front counter for pickup by the requestor
Docket Certificate:
Scan and save the certificate and attachments as one PDF document.
