The Reaffirmation Cover Sheet (Official Form 427) provides information necessary for the court to determine whether the reaffirmation agreement is presumed to be an undue hardship for the debtor under 11. U.S.C. § 524(m). The party should file the cover sheet and agreement as one document (pdf file) or file the cover sheet as an attachment to the agreement. When the party fails to include a cover sheet with the agreement, the filer must file the document using the below event. |
