A Bankruptcy Petition Preparer must complete and file the below forms whenever they help prepare documents for a debtor. If more than one bankruptcy petition preparer assists with the documents, each preparer must sign Part 2 of the Official Form 119. A Bankruptcy Petition Preparer who does not sign the form may be fined, imprisoned, or both. See 11 U.S.C. § 110 and 18 U.S.C. § 156. Required Forms: Official Form 119 Bankruptcy Petition Preparer's Notice, Declaration, and Signature. See 11 U.S.C. § 110(h)(2) Director's Form 2800 Disclosure of Compensation of Bankruptcy Petition Preparer. See 11 U.S.C. § 110(h)(2) The Bankruptcy Petition Preparer must complete and sign the forms and file with the petition. |
