The debtor must file their chapter 13 plan using the local form Chapter 13 plan. The debtor (debtor's attorney) must serve a copy of the Chapter 13 plan and the local form notice. The form plan allows the debtor to include:

Motions for valuation;
Motions to avoid liens;
Request to surrender collateral; and
Request to assume executory contracts and/or unexpired leases.

Docketing Process:

The filer must select the specific reliefs (if any) included in the plan.
