Chapter 11 Certification for Discharge

11 U.S.C. § 1192

An individual Chapter 11 debtor must make specific certifications to receive a discharge. i.e., Completion of plan payments 11 U.S.C. § 1141(d)(5)A); domestic support obligations 11 U.S.C. § 1129(a)(14); and Section 522(q) 11 U.S.C. § 1141(d)(5)(C).

A certification form is available on the court’s website, www.mssb.uscourts.gov. (Local Form: MSSB-4004-Cert - Chapter 11 Individual Debtor Certification and Request for Discharge.) 

The debtor may attach the certification to their Motion for Entry of Discharge or file it separately.  See Motion for Entry of Discharge.
