Unsworn Declaration Under Penalty of Perjury

Fed. R. Bankr. P. 1008

28 U.S.C. § 1746

The debtor must include an executed unsworn declaration under penalty of perjury ("Statement") with the petition, lists, schedules, statements and amendments thereto. See Fed. R. Bankr. P. 1008

“I declare (or certify, verify, or state) under penalty of perjury that the foregoing is true and correct. Executed on (date) (Signature)”. See 28 U.S.C. § 1746(2)

When filing the original or amending multiple documents, the debtor may include the Statement as the last page of the filing.
